How can I replace one of the SAN domains that I have already activated?

If you have already enabled SAN Domains and want to replace one of them, follow the procedure for your certificate authority:

GeoTrust - Comodo - Thawte:

1.In case that among the domains you want to replace is the primary SSL domain, for which you had exported the CSR, you will need to issue a new CSR. If your main domain remains and you want to replace the rest, this procedure is not necessary.
2.Then contact Top.Host support to provide us with the new CSR, if necessary, as well as the domains you want to replace. Then the technician will proceed to add and reissue your certificate.

Symantec:

1.You should reissue the CSR.
2.Then proceed to reissue your SSL, through the panel της Symantec.
